Position: Closing The Gap Integrated Team Care Aboriginal & Torres Strait Islander Outreach Worker
Working Location: Position available to be based in Marion with the expectation to attend all sites as required.
Salary:  Remuneration will be negotiated dependent on skills, experience and qualifications.
Status:  Full Time Fixed term with the likelihood of extension
Closing Date: 13th January 2022

Learn about the position:
The Aboriginal and Torres Strait Islander Outreach Worker (ATSIOW) will work closely with other CTG ITC team members to better connect Aboriginal and Torres Strait Islander peoples to health services that will improve the health outcomes of Aboriginal and Torres Strait Islander peoples and reduce the barriers to access appropriate primary health care services.

Reporting directly to the CTG ITC Team Leader, your responsibilities will be:

  • Establish links with local Aboriginal and Torres Strait Islander communities to encourage and support the increased use of primary health care services in their region
  • Provide practical assistance to Aboriginal and Torres Strait Islander people in attending appointments and to access other health services as required, including follow up care, specialist services and community pharmacy.
  • Work with the CTG ITC team to identify barriers that may be restricting Aboriginal and Torres Strait Islander access to other health services, and in conjunction with the Indigenous Health Project Officer, work with local Aboriginal Health Services and the local community to implement solutions
  • Liaise with health providers including GP’s, Nurses, Allied Health, Specialist, Aboriginal Health Workers and other Primary Health and administrative staff to support the successful implementation of referral pathways

Experience requirements:

  • Demonstrated experience working within a fast-paced environment
  • Demonstrated experience in engaging with Aboriginal and Torres Strait Islander community groups and individuals.
  • Relevant training and experience in a relevant health or community services fields.

Qualification requirements:

  • Knowledge and experience in using the Microsoft suite of software and databases
  • Certificate level qualification in Aboriginal and/or Torres Strait Islander primary health care (or willingness to undertake such training funded through the Closing the Gap package)
