Both positions are Indigenous Mental Health Care Coordinators, fulltime. The first will be located either in Armidale or Glen Innes and the second in Tamworth.

Indigenous Mental Health Care Coordinator

· Based in our Armidale or Glen Innes office
We currently have an exciting opportunity available for an Indigenous Mental Health (IMH) – Care Coordinator to join our team.
Located in our Armidale or Glen Innes office, and servicing the Tablelands cluster with transport provided, you will work closely with other members of the IMH Team, as well as the broader HealthWISE team to achieve the outcomes of this program in an effective and patient focussed manner.

The successful applicant will work closely with numerous stakeholders, including other members of the HealthWISE team, community services agencies, allied health providers and particularly with General Practices in working toward the delivery of a holistic health care approach to Aboriginal & Torres Strait Islander community members.
